To qualify for medical, life and disability benefits you must be a full-time Team Member who works at least 30 hours per week on a regular weekly basis.
ELIGIBLEEligible Dependents:
Spouse
Children up to the age of 26 regardless of student status
Biological, adopted, step-children, legal guardian, or legal ward children
Children over the age of 26 who are physically or mentally incapable of self-support (as determined by the claims administrator)
A grandchild who is a dependent of the employee for federal income tax purposes

During Open Enrollment
To update your medical benefit elections, use the Open Enrollment Form and elect/update the medical plan and number of dependents you want to be covered.

CHANGESHow to Make Changes
Any benefit elections that you make will remain in effect during the entire plan year. The plan year begins 1/1/2022 and ends 12/31/2022. During the plan year, you may change your benefit elections if you have a qualified family status change as determined by the IRS.
Qualified changes in status include marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of a spouse, child, or other qualified dependents, commencement or termination of adoption proceedings, or change in spouse’s benefits or employment status.
If you experience a qualified status change, you must notify Payroll and Benefits within 30 days of the event in order to make changes to benefits.
